Can Vulvodynia Feel Like a UTI?

So What Is Vulvodynia?
Vulvodynia is chronic pain or discomfort in the vulvar region without an identifiable cause like infection or skin condition. It can feel like burning, stinging, rawness, or irritation, and it can be constant or triggered by things like sitting for long periods, tight clothing, or sexual activity. Some people describe it as feeling like they always have a UTI that never quite goes away.
The overlap in symptoms is the biggest source of confusion. Both conditions can cause burning with urination, urgency, and pelvic pressure. The difference is that vulvodynia is not an infection. It is a pain condition, and treating it like an infection with antibiotics is not going to help.
Why Does This Keep Getting Missed?
Part of the problem is that vulvodynia is not something that shows up on a standard urine test or pelvic ultrasound. It requires a clinician who is specifically looking for it and knows how to assess for it. A lot of people spend years cycling through antibiotics, being told their labs are normal, and wondering what is wrong with them.
The other piece is that the pelvic floor is almost always involved. The muscles, nerves, and connective tissue in the pelvic region are deeply interconnected. When there is chronic irritation or pain in the vulvar area, the pelvic floor often responds by guarding and tightening, which creates its own layer of symptoms on top of the original problem. Urgency, pressure, and burning with urination can all come from pelvic floor muscle tension, not just the bladder or urethra.

Is there nerve sensitivity in the vulvar region that is driving the burning? Is the pelvic floor holding tension that is contributing to urgency and pressure? Are there postural or hip mechanics that are loading the pelvic nerves in a way that makes symptoms worse? Oftentimes there is also a hormonal component, especially in perimenopause or postpartum, where tissue changes make the vulvar region more sensitive and reactive.
